Eligibility

Eligibility

  • No insurance or major medical with high deductible
  • Income at or below 250% of Federal Poverty Guidelines
    household size of 1 = $27,925
    household size of 2 = $37,825
    household size of 3 = $47,725
    household size of 4 = $57,625
    household size of 5 = $67,525
    household size of 6 = $77,425
    household size of 7 = $87,325
    household size of 8 = $97,225
    add $9,900 for each additional household member
  • Age
    35-45* Cervical screening services (also includes clinical breast exam)
    45-64   Breast and cervical screening services

    *Exception: women 35-45 who have a current breast concern are eligible for both breast and cervical screening services through WWWP. Women under 35 may be eligible for breast services through the Treatment Access Fund.

    No US citizenship requirement for WWWP

Covered Services

Please note

If you choose to have services not covered by WWWP at the time of your appointment you will be responsible to pay for those services.

Office Visits

(Only when related to screening for WWWP services)
  • Preventive Medicine New Client Office Visit
  • Preventive Medicine Established Client Office Visit

Breast Services

  • Clinical Breast Exam
  • Mammogram (screening & diagnostic)
  • Breast Ultrasound
  • Other breast biopsy services may be covered with prior approval. Please contact the WWWP office at (608) 242-6392 immediately if you need additional breast services.

Cervical Services

  • PAP Test (screening & diagnostic)
  • Thin Prep PAP Test
  • Pelvic Exam
  • Colposcopy (with or without biopsy)
  • Other cervical biopsy services may be covered with prior approval. Please contact the WWWP office at (608) 242-6392 immediately if you need additional cervical services.

WWWP does not pay for blood tests or prescription medication of any kind

Two ways to apply

Please note

Enrollment must be completed and approved through a WWWP office prior to contacting any WWWP provider for screening services

Applicants must enroll through the WWWP Office in their county of residence (PDF)

  2. The WWWP Enrollment Form can be printed off and mailed or faxed to:

Once WWWP receives your Enrollment Form, a case manager will contact you to complete the enrollment process. You are not enrolled in WWWP until you speak with a WWWP case manager and have your application approved.
