PHMDC Immunization Clinics
Immunizations are one of public health's greatest achievements. Vaccines help prevent diseases and help keep people of all ages healthy. Public Health offers FREE immunizations to uninsured adults and children and children with Medical Assistance.
New Schedule! Appointments Only

Beginning January 1, 2014, immunizations will be done by appointment only. Clinics will be held in Madison (South and East), Sun Prairie, and Middleton but with some changes to time and frequency.

Things you should know

  • Immunizations are provided free of charge.
  • Beginning January 1, 2014, immunizations will be done by appointment only. Call (608) 266-4821.
  • If you are more than 15 minutes late for your appointment, you may be rescheduled.
  • Bring all immunization records to each clinic.
  • Interpreter provided.
  • An immunization clinic will be cancelled if the public schools close in the community where the clinic is scheduled.
  • Parent must attend clinic with child. If this is not possible, discuss concerns when making the appointment.
  • Attention Foreign Exchange Student Host Families: Please let us know when making the appointment that you are the host family. Special arrangements for parental permission need to be made before the clinic appointment.
Who can get immunizations through Public Health?

Public Health Madison & Dane County can provide free immunizations to children through age 18 years who:

  • Do not have insurance, or
  • Do have Medical Assistance or Badger Care, or
  • Have 'major medical insurance' (only covering hospitalization; no preventive care coverage), or
  • Are Native American or Alaska Native

If you have health insurance that covers vaccines, call your provider to be immunized.

  • PHMDC can no longer immunize children or adults who have private health insurance covering vaccines, even if they must meet a deductible or have a co-pay.
  • Call your insurance company to learn what vaccines are paid for with your policy.

Both the City of Madison and Dane County have adopted policies which prohibit firearms in buildings that they own, lease or control. This includes Public Health offices and clinics.
