What Immunizations are Offered?
Free Immunizations for Children Through Age 18
- Polio
- Diphtheria-Tetanus-Pertussis (DTaP) and (Tdap)
- Tetanus-Diphtheria (Td)
- Haemophilus b (Hib)
- Rotavirus
- Meningococcal
- Pneumococcal conjugate (PCV) to age 5
- Measles-Mumps-Rubella (MMR)
- Hepatitis B
- Hepatitis A
- Varicella (chickenpox)
- Influenza (seasonal)
- Human Papilloma Virus (HPV)
Free Immunizations for Adults
Adults who may get immunized at PHMDC:
- Uninsured for vaccines
- No Medical Assistance (MA) for vaccines
- Must meet other criteria for certain vaccines
Tdap (Diphtheria-Tetanus-Pertussis)
All adults, regardless of insurance status:
- Adults should receive one dose of Tdap instead of a Td dose.
- If you will be in contact with an infant it is especially important that you get Tdap to protect the baby from pertussis (whooping cough).
Measles Mumps Rubella (MMR)
- All uninsured who need it
Varicella (Chickenpox)
- Uninsured with no history of disease
Hepatitis A and Hepatitis B vaccine

Vaccines available through Merck Vaccine Patient Assistance Program. (You do not have to be a citizen.)
Hepatitis B-all ages
HPV (Human Papilloma Virus) women and men age 19 through 26 years
To qualify for the program must have:
- Income less than:
- $44,680 for individuals;
- $60,520 for couples or
- $76,360 for 3
- No health insurance that covers vaccines.
Prior authorization is needed before attending one of the regularly scheduled immunization clinics.
- Complete the Merck Vaccine Patient Assistance Program (PDF) Application, Section 1 only (or call 266-4821 to request a copied mailed). All Applicant Information needs to be complete and signed in 2 locations on form.
- Mail or drop off completed application to: Public Health MDC, 2705 E. Washington Ave., Madison, WI 53704
- Public Health staff will process the application to the vaccine manufacturer. Staff will call client to confirm the approval. The application is valid for 30 days at any of our regularly scheduled walk-in immunization clinics. This process must be completed for each dose of the vaccine.
