Phmdc immunization clinics

The full range of childhood immunizations, as well as some adult immunizations are provided free of charge at Public Health Immunization Clinics in Madison, Middleton, Verona, Stoughton and Sun Prairie. Vaccines are available to prevent many diseases that can have serious complications and may even be fatal.

How Do I Get an Immunization?

  • You may attend any clinic listed in the schedule.
  • Immunizations are provided free of charge. (For those whose insurance covers immunizations, it is preferred that they see their own health care provider.)
  • No appointments are needed except for the Adult HPV Clinic at the Madison East site. See guidelines for this clinic under Free Immunizations for Adults.
  • Bring all immunization records to each clinic.
  • Interpreter provided.
  • Children under 18 must be accompanied by a parent or legal guardian.

What Immunizations are Offered?

Free Immunizations for Children Through Age 18

  • Polio
  • Diphtheria-Tetanus-Pertussis (DTaP) and (Tdap)
  • Tetanus-Diphtheria (Td)
  • Haemophilus b (Hib)
  • Rotavirus
  • Meningococcal
  • Pneumococcal conjugate (PCV) to age 5
  • Measles-Mumps-Rubella (MMR)
  • Hepatitis B
  • Hepatitis A
  • Varicella (chickenpox)
  • Influenza (seasonal)
  • Human Papilloma Virus (HPV)

Free Immunizations for Adults

  • Td/Tdap
    • Adults should receive one dose of Tdap instead of a Td dose.
    • If you will be in contact with an infant it is especially important that you get Tdap to protect the baby from pertussis (whooping cough).
  • Hepatitis A, Hepatitis B, Varicella and MMR vaccines are available only to adults who fall into certain categories.
  • HPV causes serious diseases such as cervical and anal cancers and genital warts. About 20 million Americans are currently infected with HPV and most don't know they have it. Over half of sexually active men and women will get HPV disease some time in their lives. The vaccine, a series of 3 shots, is available to qualifying women and men age 19 through 26 years:
    • Income less than:
      • $43,560 for individuals;
      • $58,840 for couples or
      • $89,400 for a family of four
    • No health insurance that covers vaccines.
    • You do not have to be a citizen.
  • Prior authorization is needed before attending one of the regularly scheduled immunization clinics to receive the HPV vaccine.
    • Complete the Merck Vaccine Patient Assistance Program (PDF) Application, Section 1 only (or call 266-4821 to request a copied mailed). All Applicant Information needs to be complete and signed in 2 locations on form.
    • Mail or drop off completed application to: Public Health MDC, 2705 E. Washington Ave., Madison, WI 53704
    • Public Health staff will process the application to the vaccine manufacturer. Staff will call client to confirm the approval. The application is valid for 30 days at any of our regularly scheduled walk-in immunization clinics. This process must be completed for each dose of the vaccine.

  • Public Health does not give vaccines for international travel.
  • Any vaccine series begun by a public health agency will be completed free by public health.
