Phmdc immunization clinics
The full range of childhood immunizations, as well as some adult immunizations are provided free of charge at Public Health Immunization Clinics in Madison, Middleton, Verona, Stoughton and Sun Prairie. Vaccines are available to prevent many diseases that can have serious complications and may even be fatal.

How Do I Get an Immunization?
- You may attend any clinic listed in the schedule.
- Immunizations are provided free of charge. (For those whose insurance covers immunizations, it is preferred that they see their own health care provider.)
- No appointments are needed.
- Bring all immunization records to each clinic.
- Interpreter provided.
- Children under 18 must be accompanied by a parent or legal guardian.

- Polio

The following vaccines are FREE for adults during 2010:

how many shots, and why do i need them?
Tdap - Tetanus, Diphtheria, Pertussis
- One dose, if it is more than 5 yrs since your last tetanus vaccine, age 19-64.
- Especially if you have contact with any infants, and it has been 2 years or more since you had a tetanus shot. This helps protect the baby from whooping cough disease.
Hepatitis A
- 2-dose series, any age.
- Especially for adults who have contact with a recently adopted child from a country where Hepatitis A is common, have chronic liver disease, men who have sex with men, people who use street drugs.
Hepatitis B
- 3-dose series, any age.
- Especially for people who use injection drugs, men who have sex with men, history of a sexually transmitted disease, immigrant from an area of the world where Hepatitis B is common, work with blood/ body fluids, or people with developmental disabilities, or have chronic liver disease.
HPV - Human Papilloma Virus
- 3-dose series, 19-26 yrs.
- Helps prevent cervical cancer and genital warts in women.
- Prevents genital warts in men.
Varicella
- 2-dose series, any age.
- If you never had chickenpox disease.
MMR - Measles, Mumps, Rubella
- 1 dose if you've never had MMR vaccine and if you were born in or after 1957.
- 2 doses if you are a health care worker or student in college and have no evidence of immunity, any age.
