SURVEILLANCE: UNDERSTANDING OUR CHRONIC DISEASE HEALTH RISKS
- Analyze disease and risk factor data to understand trends, how Dane County rates compared to other places and national health goals, and identify groups at higher risk
- Use data to target public health programs
- Conduct and analyze population and neighborhood-level health assessments and surveys of groups in order to plan effective, targeted interventions
- Provide surveillance data to community organizations and partnerships, to inform community efforts to address health risks
- Produce chronic disease related reports and disseminate important findings
